Submitting a dispatch request or route assessment form is a request for service, not a guarantee of acceptance. All requests are subject to availability, service window, and territory review before confirmation.
FJ Enterprise Medical Courier provides after-hours scheduled and STAT/on-call courier service during the 6 PM to midnight coverage window, seven days a week. Requests outside this window may be reviewed manually and accepted only when available.
STAT requests are subject to availability at time of receipt. Acceptance is not guaranteed. Service tier is determined by notice period at time of receipt — any request received with less than four (4) hours notice is automatically classified and invoiced as STAT service, regardless of how the request is labeled by the client. This classification is not subject to dispute after service is rendered.
All pricing is confirmed in writing before the first run. Final invoice amounts may vary based on actual stops, wait time, parking, tolls, access delays, after-midnight dispatch, frozen specimen handling, and long-distance diversion. Published minimums represent the base starting rate only.
No Protected Health Information (PHI) may be submitted through any website form. This includes patient names, dates of birth, accession numbers, specimen IDs, diagnosis, test names, insurance information, or medical record numbers. Only generic specimen categories, counts, temperature requirements, pickup/delivery details, and access instructions may be submitted.
The client is responsible for ensuring specimens and packages are properly packaged, labeled, and ready for pickup at the agreed time, unless otherwise specified in a written service agreement.
FJ Enterprise Medical Courier does not provide medical advice, clinical interpretation, specimen testing, or diagnostic services. We are a transport-only service.
Proof of delivery, chain-of-custody documentation, and temperature logs may be provided when applicable and agreed upon in the service agreement.
