Do Not Submit
- Patient names
- Dates of birth
- Accession numbers
- Specimen IDs or lab order numbers
- Diagnosis or clinical information
- Test names or test results
- Insurance information
- Medical record numbers
- Any other information that identifies a specific patient
You May Submit
- Generic specimen category (e.g. "blood specimens", "body fluid specimens")
- Specimen count and container count
- Temperature requirement (room temperature, frozen, mixed)
- Pickup facility name, address, and department
- Delivery facility name, address, and department
- Access instructions (e.g. "enter through ED, call security")
- Requested pickup time and delivery deadline
- Callback or confirmation request
- Test submission indicator (write "TEST" in notes if applicable)
Why This Matters
Our website form is transmitted over a standard web connection and stored in operational tools not certified as HIPAA-compliant data systems. Submitting PHI through this form creates an unnecessary compliance risk for your organization and ours.
